REGISTRATION/ACCEPTANCE FORM

Please fill in the information below. You'll have to choose a Festival and submit a contact E-mail as a copy of the information you submit will be E-mailed to you. Please submit separate forms for Junior and Senior Honor Orchestra. If more than ten students are attending, you will have to submit a second form.

Submission of this form certifies that all the students accepting are in good standing with the music department at our school and have completed the Festival Acceptance Agreement which has been signed by their parents and is on file with me.

I understand that I will be responsible for all local arrangements for participation, including but not limited to musical preparation, travel arrangements, fee payments, and communication with the executive consultant and the parent community.

I understand that I am required by AMIS to make travel arrangements that permit all participants to be on time for the first event of the Festival.

I understand that when housing festival participants with host families, it is often possible to place two or more students with the same family. However, I also understand that this is not always possible, as some families have room for only one guest. I accept that staying with host families in other parts of the world is an important part of the AMIS philosophy, and a major part of the total festival experience in which I am to participate.

Our school understands that a student may not participate in any festival unless accompanied by his or her director. If the director is unable to attend the festival, my school will inform the Executive Consultant of the approved substitute chaperone.

I understand that students from schools that are AMIS members as of November 1, 2007 pay "member" fees. Students from schools other than those that are AMIS members as of November 1, 2007 pay "non-member" fees.

I further understand that teachers from AMIS member schools (as of November 1) pay "member" rates. Teachers who are individual members of AMIS also pay "member" rates. All others pay "non-member" rates.

I am aware that only students from AMIS member schools are eligible to apply for financial aid.
