IB MUSIC WORKSHOP
REGISTRATION/ACCEPTANCE FORM

Please fill in the information below. You'll have to submit a contact E-mail as a copy of the information you submit will be E-mailed to you. If more than fifteen students are attending, you will have to submit a second form.

Submission of this form certifies that all the students accepting are in good standing with the music department at our school and have completed the Festival Acceptance Agreement which has been signed by their parents and is on file with me.

I understand that I will be responsible for all local arrangements for participation, including but not limited to musical preparation, travel arrangements, fee payments, and communication with the executive all participants to be on time for the first event of the workshop.

I understand that I am required by AMIS to make travel arrangements that permiterstand that when housing workshop participants with host families, it is often possible to place two or more students with the same family. However, I also understand that this is not always possible, as some families have room for only one guest. I accept that staying with host families in other parts of the world is an important part of the AMIS philosophy, and a major part of the total workshop experience in which I am to participate.

Our school understands that a student may not participate in any workshop unless accompanied by his or her director. If the director is unable to attend the workshop, my school will inform the Executive Consultant of the approved substitute chaperone.
