TRACK LIST AND ENTRIES FORM

Please fill in the information as requested below.

List your students in the same order that they appear on the individual instrument or voice part cd/mini disc/cassette tape that you are submitting. Please label each of your recordings with the instrument/voice part that is on that cd/mini disc/cassette tape. Do not label with your school's name. Use the audition code for your school.

International Honor Orchestra auditions should fill in one form per instrument for JHO and one form per instrument for SHO. Write clearly BOTH on the CD/Tape and on the case: Jr. or Sr. IHO, instrument, and your school audition code.

Print a copy of this page OR print a copy of the confirmation E-mail sent to you and enclose it with your auditions. If a copy of this web page or E-mail is not submitted with your recordings, your auditioning students will not be evaluated by the audition committee.

You must submit a contact E-mail since a copy of the information you submit will be E-mailed to you. Please keep this for your records.

If you have more than twenty entries for any one instrument/voice, please submit the online form, reset it, and submit a second form with the balance of the entries. You only need to put the school address, home and school telephones, and fax on your first entry.
